CENTER FOR HEALTHY AGING

Aging is a normal dynamic process and not a disease.  It is part of the continuum of life.  Disabling conditions that often accompany aging can be prevented.  Productive and active aging can be enhanced.  This is achieved through healthy lifestyle, by engaging in health promotion activities, and regular visits to your physicians.

Presence of medical conditions, however, need not prevent you from achieving wellness.  Complete well-being – physically, mentally and spiritually – can be maximized in the context of each individual’s functional and psychosocial situation, in a dynamic process unique to the individual.

At the Center for Healthy Aging, our goal is to help older adults lead healthier and more satisfying lives through patient and family –centered care.  Our doctors are trained and certified in Geriatric Medicine, the specialization that caters to those aged 60 years and older.  Together with the other staff members, we work for and with older adults towards a life of infinite possibilities.  This is our commitment – Continued Health, Active, Meaningful Partnership.  This is our special interest.  This is our passion – CHAMP.

Service Offerings

  • Health consult for geriatric assessment
    • Comprehensive health assessment of seniors aged 60 and older
    • Monitoring health conditions
  • Individual and / or family and caregiver counseling
  • Immunization
  • Nutritional supervision
  • Prescription assistance for polypharmacy (or too many medicines)
  • Health screenings and other health promotion packages
  • Homecare services (to include physicians visits)
  • Support groups
  • Advocacy on issues related to elderly care

Availment of Services

  • The Center for Healthy Aging serves all persons aged 60 years and older.
  • Primary attending physicians may refer their ambulatory patients to the Center for geriatric assessment and/or other special needs.
  • The Center gives copies of the clinical summary report to the patient (and to the referring physicians, upon the consent of the patient).
  • The Center gives patients age-appropriate information on mental health, nutrition, exercise and other relevant information.

  • Multiple medical problems
  • Memory problems such as Alzheimer’s Dementia
  • Movement and mobility problems such as Parkinson’s Disease
  • Stroke
  • Osteoporosis
  • Falls and fractures
  • Polypharmacy
  • Pain
  • Weight Loss
  • Loss of sleep and appetite
  • Incontinence (unable to control urine or stool)
