1ST POSTGRADUATE COURSE IN CRITICAL CARE PHARMACY

The Medical City Pharmacy Department and the Section of Critical Care Medicine invite you to the 1st Postgraduate Course in Critical Care Pharmacy on April 12-14, 2014, 7:00 am to 5:30 pm. at Augusto M. Barcelon Auditorium, 6th Floor, Podium Building, The Medical City.


The Medical City Pharmacy Department and the Section of Critical Care Medicine invite you to the 1st Postgraduate Course in Critical Care Pharmacy on April 12-14, 2014, 7:00 am to 5:30 pm. at Augusto M. Barcelon Auditorium, 6th Floor, Podium Building, The Medical City. The registration fee to cover the 2 ½ days of lecture is P 3,500 inclusive of meals.

Multi-disciplinary rounds in the Intensive Care Unit and the Acute Stroke Unit for pre-selected participants will be held in the afternoon of 14 April 2014. To maximize the learning experience, slots have been limited to 10. Selected participants will be informed regarding the acceptance via phone call or email on or before April 10, 2014.  The fee for this immersion activity is Php 1,000.

April 10-11   TMC Pharmacy Office (9th Floor, Nursing Tower 2 Bldg.)
April 12        on-site

Print 2 copies of the completed registration form and bring to the Cashier's Office at The Medical City for payment.  Your slot will only be reserved upon payment.

 

PAYMENT OPTIONS

Payment can either be Cash or Check. If Check, please make it payable to "Professional Services Inc."

If your company will shoulder your course fee, please provide us with BIR Form 2307 (Creditable Withholding Tax Certificate) as the cost will be net of tax.

Registration forms are also available at the Pharmacy Department Office (9th Floor,Nursing Tower 2) if there will be problems in completing and/or printing the form online.

Present your copy of the registration form, the official receipt, and a valid ID on April 12, 2014.
